Replacement cards are only offered to individuals who have passed the programs provided by PALScourse.com. If your
card was lost, stolen or not reported that it wasn't received within 30 days of passing the program, you can now order
a replacement hard copy card. If you have provided an incorrect shipping destination or name, you must also order
a replacement card. In certain circumstances, our staff may be willing to send a replacement card depending on the
nature of the issue. If you feel you qualify for this, please explain in the form below.
